The company is owned by Dr. Evan Cantini and Dr. Alvin McLean.  The executive team has over 100 years of combined healthcare experience. There is no diagnosis too complicated for our Executive Team which consists of a Rehabilitation Physician, a Neuropsychologist, an RN, and a PT.

The core principles on which CHC Services operates include rigorous ongoing assessment and problem identification, individualized care plan development, meticulous training, supervision and certification of homecare staff, collaboration with the client’s loved ones and healthcare providers.

  • Care Management (CM) Services:
  • The Care Manager helps individuals and families manage the complex needs related to health issues through the development of a Care Plan. The CM advocates, coordinates, communicates, and identifies necessary resources for the client and family while promoting quality, cost-effective interventions and outcomes.
  • Companion Services:
  • Companion services are provided by specialty trained Care Specialists. They help individuals meet their everyday needs safely and effectively. The Care Specialist activities are driven by the Care Plan and may include: personal care, toileting, assistance with medication, physical and cognitive support, simple housecleaning, meal preparation, driving, recreational activities and exercise programs.
  • Skilled Healthcare Services:
  • Skilled healthcare services include nutritional counseling and therapy services, such as PT, OT & Speech.
