Care Management Associates, a division of Connecticut Community Care, Inc., provides in-home assessments statewide, phone and office consultations on elder care options, and quality home care services.  Our Personal Assistants are companions, homemakers, and Certified Nurse Assistants who help our clients with meal preparation, companionship, housekeeping, shopping, transportation, pet care, managing mail, personal care duties, appointment scheduling, leisure activities and more!  Experienced social work and nurse care managers oversee each person's plan of care and home care services.  We also provide additional services such as ongoing care management, money management, personal chef services, counseling, therapeutic massage, probate court expert witness, personal records organization, and assistance with relocation. 

Our organization is a private-non-profit agency that has provided care management to older adults and people with disabilities for over thirty years.  In 2007 we were voted "Best in Home Care" by the readers of the Connecticut Law Tribune.
