Silverado Senior Services provides specialized
Home Care and Care Management for individuals with memory care needs. Services begin with a Comprehensive Assessment, which includes a detailed, written report that can be shared with physicians, family or others involved in the individuals care. Silverado Senior Services then works in conjunction with the physician, client, family, and others to create a highly specialized, comprehensive Care Plan. This is an ongoing process and the Geriatric Care Manager will work with the group to ensure changing needs are being met.
Care Management includes coordination of such needed services as financial planning, family counseling and assistance, rehabilitation services and long-term planning. A consultation with a national financial planning firm is also available at no extra cost.
Home Care Services are directed by Silverado Geriatric Care Managers. The
Home Care Companions are specifically and specially trained to handle the unique and often complex needs of individuals with memory impairment and are managed by the Geriatric Care Manager.
Silverado Senior Services is part of a full continuum of services available, including award winning
Assisted Living communities and compassionate, expert Hospice Care. These additional services are available if or when needed and can be coordinated by your Care Manager.
